The convention is open to all, members of the ASA or not. Non-members who wish to join for $25 will receive a welcome packet including the most recent issue of The Azalean, and will have their membership extended through 2004.
1 Please print your information clearly. It only counts if we can read it.
2 Mark an X (*or a V to request vegetarian meals) in the column below each person's number, one line for each event they wish.
3 For each line:    - write the total number of marks under the # heading, and    - multiply that number by the Price per person and write the result in the Total column.
4 Add up the Total column and write that result on the bottom Total line.
